Make sure the distribution was in 2021. The Form 1099-R reports a lot of information about your retirement plan or IRA distribution that will be required in order to be sure that the correct amount is being taxed on your return.
You need to contact the financial institution that would have sent you the Form 1099-R and ask for a replacement copy.
If you have an online account to manage your distributions, the Form 1099-R may be available to you online. Look for tax forms or year end statements. You may try and import it, also. Click here for instructions.
