Never file my taxes before, made a mistake on my last name and forgot to mention about my Roth IRA contribution. What to do?

might not matter as to name because you provided ssn.     you don't tell the IRS about a Roth unless excess contribution.  was it efiled and accepted.  if so really nothing to do unless you were due a refund and you filed more than a month ago.  then you can go to irs website to check status or call them 800-829-1040 - option 2
