My employer was out of compliance w/ 401k requirements so I was issued compliance correction check. This is a pre-tax amount, how do I report it and pay taxes on it?

Was it include on your W-2 of did you get a 1099-R.

It may be best to contact the employer and determine haw it was reported by them.
