My CSA-1099R box 5 only shows my Medical Insurance premiums. Can I add my Vision and Dental Insurance premiums to the box 5 total and subtract that amount from box 9b?

No. Report that form exactly as you see it. Other medical expenses can be deducted as an itemized deduction if you itemize.
