IRS form 56 Notice Concerning Fiduciary Relationship - how to fill it properly

My father passed away in 2020. He retired from University of Califonia (UC). UC retirement administration center required me to submit form56 in order to provide 1099-R to me. 

 

I have the following questions regards to form56

1.  Under Part I Identification - There is a Identifying number section,  I am not sure what number I should use here. I received a EIN number from IRS for my dad's estate, should I enter EIN number as identifying number?

2.  Under Section B - Nature of Liability and Tax Notices

      3. Type of Tax - I selected income, Estate

      4. Federal tax form number (check all that apply):
           e. 1040 or 1040 SR,
           f. 1041,
          h. other - I entered 1099-R
Could you let me know if my entry on form 56 is correct?
If you have additional suggestions, please let me know.

Enter the estate's EIN and you do not need to enter "1099-R" as shown in your screenshot.
