Received lump sum annuity distrib when father died. Earnings taxable.Insurance co. didn't send 1099-R, just email with tax cost basis & taxes. Where to enter inTurboTax

I tried creating a substitute 1099-R with the email information, but I don't have a federal and state ID for the insurance company, which are both required. Is there someplace else to enter this correctly?

If this information is required to be reported on a 1099-R, there is no other place to enter the information on your return, but without an ID number, you can't file your return. Your first step should be to contact the insurance company and find out why they did not send you a 1099-R.  It may have been misdirected or lost in the mail.  

If they did not send you a 1099-R on purpose, you can find out the reason why, and that may help determine if it should go in another area of your return.
