If my husband and I filed jointly, do I have to add him to my tax addendum or should it just be under my name? I am the one who received a 1099 form.

An amended tax return must have the same names and same order of names that the original return had. If you do it in TurboTax and did your original return in TurboTax, the names will automatically be entered on the amended return form 1040X.
