If I can't find my 401k form what do I do

I've lost my 401k form and don't have the info as to exactly how much I Wasatch paid or the form itself. What do I do?

Are you talking about money that you took out of your 401(k)? Contact the administrator of your 401(k) plan for a duplicate copy of the Form 1099-R showing the distribution.
