Do NOT enter a zero in the TT program for box 2a ... instead leave the box blank and complete the follow up screens ...  OR if the entire distribution is taxable then just enter the box 1 amount in box 2a ... either way will get the same result.

I tried both of those, and nothing changed. I didn't make a lot of income this year, but I feel like this should still have an impact on my refund, etc... Thoughts?

It appears that you requested a nontaxable direct rollover to the IRA, but failed to complete the direct rollover.  This means that the code G Form 1099-R issued by the payer to report the direct rollover does not reflect what actually happened.

Your only option is to submit a substitute Form 1099-R (Form 4852) showing in box 2a the amount in box 1 minus any amount that might be in box 5, and code 1 or 7 in box 7 depending on whether or not you were under age 59½.  To do so you must first contact the payer to request a corrected Form 1099-R, but under the circumstances the payer will decline since they did what you asked.  You can then complete the substitute Form 1099-R with explanations of your attempt to obtain the corrected Form 1099-R and the reason that the original was incorrect.  Filing a substitute Form 1099-R requires that you mail your tax return, you are precluded from e-filing.
