I need to report an annuity cashing out

Two distributions were taken from an annuity in 2018.  The 1099-R states 7, D in line 7.

There are 2 entry boxes for Box 7.  Enter the 7 in the first, and the D in the second.  They don't both go together, the spaces are just very close together on the form but clearly separated on the program screen.
