I'm trying to add a 1099-B, and TurboTax is asking for lines 1d and 1e, but my form doesn't have that it has lines 8 and 11.

I can only guess here, of course, but assuming you really are looking at a 1099-B and not some other 1099 form, I think you are using the wrong interview here.  The "Stocks, Mutual Funds, Bonds, Other" interview is not the place to enter any and all information reported on a 1099-B.  If you've been dabbling in Section 1256 products :

• non-equity options.
• foreign currency contracts.
• regulated futures contracts.
• dealer equity options.
• dealer securities futures contracts.

then you need to use the "Contracts and Straddles" interview.
