The other 3 choices I know would not apply to me (here are all 5 choices)

took out this money due to a qualified disaster

This money was from being a public safety officer

I need to file a substitute 1099-R

The corrected box is checked on this 1099-R

None of these apply

Yes, if none of the other four choices apply to the question "Do any of these situations apply to you?", then select "None of these apply".

 

You would only select that you needed to prepare a Substitute form if your plan administrator doesn't give you Form 1099-R for 2022 (or the one you receive is wrong and your plan administrator refuses to correct it).

 

If the Corrected box is checked on your form, then select "The corrected box is checked".

TurboTax will determine from your entries whether the additional 10% "penalty" tax on early withdrawals is applicable.
