I have a form 5498 IRA contribution information: what do I do with it? No contributions were made. It is just stating the fair market value of a life insurance account

You may keep it with your files, but you do not report a 5498 on a tax return.
(you could use it as a reference if you were reporting contributions you made to an IRA account)
