I have a 1099R & am being asked for my "2016 disaster distributions from form 8915A" for 2016, 2017 & 2018 but In can't find that form on any of those year's filings.

It probably does not apply to you. These are a series of standard questions for everyone so that people that did have distributions to survive in a declared disaster area, can capture their details more easily. 

If you are uncertain about the record of distributions from Form 1099-R being reported, I recommend contacting your financial adviser to confirm.
