It depends. What determines whether you need to report the information or not is Box 1, distribution. If that number is zero, then you do not need to report the form. However, even though there is no taxable distribution, you are required to report the distribution. For instance, if the box 7 code is a G, showing a rollover, this is not taxable, but it is reportable. The same is true if it is a Roth distribution. But if for some reason you received a form stating zero distribution, this would not need to be reported on the return.
