You should have received a Form 1099-R from the insurance company or other financial institution for each annuity that was cashed in.

Enter all of the information from each Form 1099-R into the Retirement Plans and Social Security section of TurboTax. Please take care to enter the correct code(s) from Box 7 of each 1099-R.
