I already filed my taxes but just received a 1099R form. It was for a rollover and boxes 2a, 2b and 4 are 0 for taxable amounts. Do I need to worry about filing this?

The IRS might send you a bill in a year or so for tax on the box 1 amount if not reported.

If you do not amend you should keep the 1099-R handy and statements that show that the rollover was completed at the receiving end to answer the IRS letter.

If the box 7 code was a G then you probably will not hear from the IRS, but even if reported the IRS sometimes wants proof that the rollover actually occurred.

Yes the box 7 code said G. And all other boxes say 0.00 so I would assume filing an amendment would be a waste of time?

Yes, the IRS gets it from the payer and will expect to see it on your tax return.

(Personalty I would not spend the effort to amend and wait and see it the IRS questions it (in a year or two).  A phone (or written) response to any IRS letter should take care if it. Just keep your documents handy and don't dismiss it since the IRS usually takes 1-2 years to question this - they are not fast.)
