If you want to generate Form 8606 to report:
- distributions from a Roth IRA
- distributions from a traditional, SEP or SIMPLE IRA at any time after you have made nondeductible IRA contributions
- convert a traditional, SEP or SIMPLE IRA into a Roth IRA
You can mail Form 8606 separately from your tax return, so you could prepare/mail them from up to three prior tax years.
If you want to Amend a Prior Year Return to report Roth Contributions, be sure to save a copy of your originally filed return.
