How do I report a total loss on an investment in a private company?

Hi,

I invested in a non-public start-up that was liquidated and my "seed class" shares are worthless. How do I report this?  On Turbo Tax I filled form 8949 but it calls for an "attached statement" but doesn't indicate how to do so.

Any help will be greatly appreciated!
