How do I enter my roth conversion when i did not receive a 1099r because the conversion was made in the next year, but before the deadline?

How do I enter my roth conversion when i did not receive a 1099r because the conversion was made in the next year, but before the deadline?
The deadline for making a 2016 Roth conversion was Dec. 31, 2016. If converted in 2017, then you will receive the 1099-R in Jan. 2018 and it will go on yiur 2017 tax return.
The April 18, 2017 date only allows new IRA *contributions* for 2016, not conversions that are always reported in the tax year converted.
