HELP with traditional IRA distribution

I helped a friend doing her taxes last year and again this year. Her husband passed away in 2019 and filed her taxes last year 2020. (Texas so only federal taxes) Last year we filed her widowed. Now was going to file for her again for 2021 and she cashed in her dead husbands IRA for the full amount in 2021. The 1099 only has his name on it and his SS#. Taxes were withheld and I am trying to figure out if she needs to report it and how, as Turbo Tax has her to file as single. Just not sure how it should be handled.

It does need to be reported as income, however it should be reported under the beneficiary spouse's social security number at this point.  She should contact the payer (account custodian) and have them issue the correct 1099R to her.
