Form 1099R Box 14 State Tax (NY) withheld error

When you enter any number larger than zero in box 14, it is marked in yellow, and the following error message is displayed:

Form 1099-R (NATIONAL FINANCIAL SERVICES LLC): Box 14a For electronic filing, New York State withholdings can not be greater than or equal to gross distribution or state distribution. (box 1).

This prevents you from e-filing-- the program suggests mailing the return.

How will this be rectified?

To clarify, what numbers are in Box 14, 15 and 16?

Is New York the only state and is it listed only once?
