For review of a 1099R from OPM for my federal civil service retirement, over 30 years ago, I am asked to enter an annuity start date. I have no records, what do I do?

The year was 1994, I'm not sure of the month, possibly October but, not sure.

Any date in 1994 will work. It is looking to see where you fall in the tables for how to handle your income.
