The only way to fill it out is in forms mode, which is only available in the desktop TurboTax software, not in TurboTax Online. Are you reporting an excess accumulation or an excess contribution ??
If you withdrew contribution after Oct 15 2024, you missed the deadline and there is no waiver of the 6% contribution penalty for 2023.
you won't need an explanation and you don't send any other documentation.
if you withdrew after Dec 31 2024, only the 6% shows on your 2024 tax return along with an unresolved excess contribution from your 2023 Form 5329.
since you say you have no 1099-R that must be the case.

If you are reporting an excess accumulation removed in 2025, you would report that next year.

Include dates and amounts in your explanation statement.
You don't have to send any copies of checks.
