Does my "Traditional IRA" Contribution show up on my W2 in box 12 as "D"? If so, do I select the "traditional IRA" box when asked what I contributed to in 2018?

NO ... a traditional IRA contribution NEVER gets reported on a W-2 ... the code D is for a 401K contribution which is NOT the same thing.
