Yes, if you received a Form 1099-R with a distribution code of 4 in box 7, you will enter it on your tax return.

Follow the steps below to enter Form 1099-R.

1. Select Federal
2. Select Wages & Income
3. Scroll down to Retirement Plans and Social Security
4. Select Start by IRA, 401(k), Pension Plan Withdrawals (1099-R)
5. Add your 1099-R.
