Do I need to include the 1099 R from a rolled over retirement distribution on my Illinois state tax form when I submit the paper copy?

If there were state taxes withheld you would want to include copy1. See excerpt from the Illinois Revenue site.

'Attach your W-2 and 1099 forms to support the amount you claim as Illinois Income Tax withheld on Line 26'.
