Defined Contribution Plan

I received a distribution statement about a defined contribution plan withdraw.  My question is where do I enter this on my taxes.  There is no Federal ID number on it.

Defined Contribution Plan

You should receive a 1099-R for any distribution.  Besides the Federal ID number, the 1099-R will include other information such as a distribution code and information about withholdings for federal and state purposes.  If you did not receive this form, you will need to contact the Plan Administrator for a copy of this to use in filing your return.
