I am 26 yrs old and I left a job and cashed out my 401K acct for $277.96 but I did not receive a 1099-R form, how do I claim this on my taxes?

Assuming that the distribution occurred in 2019, the plan was required to provide you with a Form 1099-R by January 31, 2020. Contact the plan to obtain the missing form.
