I divided my 1099 with RMD and Conversion combined and answered all the questions . I entered a second RMD for my wife and answered all the questions. Wife's RMD of $4669 shows in list of 1099's but is not included on Sr 1040 tax form line 4a or b total for RMD Total for the combined 1099 which I divided into 2 1099's is showing on line 4a and 4b. Married is checked where asked.

To clarify, you received 2 Form 1099-R (one for your wife and one for you) and entered those into TurboTax. The distribution is more than your RMD and the extra you converted to Roth (you are not allowed to convert RMD). And the total distribution amount isn't showing correctly on line 4 Form 1040-SR? 4a should be the total distribution, 4b should be the taxable part.
Please be aware, conversions are taxable if you only had pre-tax funds.
We entered an RMD and Conversion (from contrib IRA to Roth IRA) for myself. These two show up on line 4a and 4b. Tried to then put in RMD for my wife and it went to line 5B (total income) I put them in in a different order and my wife's RMD will not go to IRA distributions line 4a or4b. Have gone over questions multiple times. This RMD does ask if it is a qualified pension (yes) . Married is checked so two RMDs should be allowed.

If you wife's distribution came from an IRA then please make sure you check the IRA/SEP/SIMPLE box when you enter your Form 1099-R:
That should fix the the issue and put it on line 4. If your wife's Form 1099-R hasn't the IRA/SEP/SIMPLE box checked and it is from a non IRA account then line 5 would be correct.
