1099-r QR code upload scrambled the address info

doing the upload with the QR code app scrambles the "recipient address" with the "payee address" so I deleted them to enter manually BUT the manual entry form doesn't accept anything about the payee but the name.  The IRS will need the payer's TIN to make the association.

How do I get that info entered now that I can't just edit the entered form worksheets directly.
