If this was not a direct rollover, and you withdraw the funds and performed the rollover yourself, then you must be sure to follow the following guidance:

After you enter your 1099-R, when asked What did you do with the money from (bank)? select I moved the money to another retirement account (or returned it to the same retirement account) and I rolled over all of this money.
