1099-R for Pension Rollover - Payer Name question - TT Desktop

First time filing a 1099-R and I have several. The first is from my former employer. The 1099-R Payer is Bank Name "As Paying Agent for" my former employer pension plan. Only the bank has a mailing address on the 1099-R. Which do I enter into Turbo Tax Desktop?
