I am resident of Maine. On 11/28/2021, I temporarily changed my mailing address from ME to NY with investment firm. Confirmation letter showed change only to mailing address, not legal address. Took my RMD in Dec with Federal tax withheld but no state tax withheld. Received 1099-R with NY as the recipient address rather than my legal address of ME. Also, Box 15 reads NY. Have been in contact with company and they will not issue corrected forms, claiming that changing my address online with them also changed my entire account address. Should the form be entered exactly as it is with NY address & NY in Box 15 although all my other tax information shows ME address.? Does the IRS share the 1099 info with the states? If so, which state, ME or NY, would have received the distribution information?

No. You should use the correct state when entering your Form 1099-R. Keep this information with your tax records. You are not required to pay tax in NY since you never lived there and the retirement is not NY income. Hopefully you will be issued a corrected form that you can keep with your return.

After more review of my tax forms, I have found 1099-Div from the same firm also with a NY address. The dividends are under $30.00 and no taxes have been withheld. Would this distribution be considered NY income requiring further reporting from me?
Since you were not a NY resident, you do not want to pay NY tax on the income. @DianeW777 is correct. The dividend goes on your resident ME return. Keep the confirmation letter showing mail address, not legal, with your tax notebook.
