The Roth Conversion part is handled differently. Even though the contributions belong to different years, the act of moving the money to Roth happened entirely in 2025. So, you will report the total amount converted.
You do not mention if you chose to make your 2025 contribution non-deductible. If so, you are performing a "backdoor" Roth conversion, you already paid tax so you won't be taxed again.
