Entering 1098t school's name.

I'm attending Minneapolis Community & Technical Collage but in 1098T it says first Minnesota State Colleges & Universities and different address after which follows Filer's Federal Identification Number. Under this number is says Minneapolis Community & Technical Collage and phone number.
I'm not sure which school name and address should I enter in 8863 form.

Use the school information from the 1098-T, although it shouldn't really matter as long as the Fed ID # is correct.
