I must fill in "Enter your 1099-R details from National Financial Services LLC as agent for but......at the Fidelity website it states I will not be getting TAX Forms this year

National Financial Services LLC is another name for Fidelity. It is hard to tell why their site said you're not getting tax form from them, when if fact you received that. You'll need to include information from the form to to you tax return.

Login to your Fidelity account and you can download your tax forms from their website.
