Form 8833 and Schedule OI

As a Chinese during OPT, which file do I use, 8843 or 8833 for Tax treaty? In addition, do I need to provide information such as those listed in schedule OI?

In addition, I know that I need to file 8843 or 8833 for federal tax return, then do I also need to file it to state tax return (california) for the $5000 tax treaty? If not, can I file federal by mail and e file state tax return?
