Received a letter from IRS requesting "amounts and dates of all estimated tax payments paid for this tax year (2016) referring to pages 1-2 or my form 1040

To respond to this letter, you will need to provide the following information:

  • Copies (do not send your originals) of any W-2s, 1099s, or other income statements that you received.  These will provide verification of both income and withholdings.
  • Copies of cancelled checks for any estimated income tax payments made for your 2016 taxes.  You should include copies of both the front and back of the cancelled check (your bank will be able to provide you with these if you do not have copies).
  • A copy of the notice (review it carefully to make sure you are including all documentation that they are requesting, and, if required, sign it).

Keep a copy in your records of exactly what was sent to the IRS.  When you mail your response, please make sure that you use a service that will provide delivery confirmation (USPS certified return receipt, priority mail, UPS, etc.)
