Turbo Tax reported that my taxes were rejected because I had checked "Form 4972ind" and had failed to attach the form 4972 which is required if you check Form4972ind. In fact I don't recall checking any such form, and cannot find it on any of my completed tax forms. As a result I did not submit form 4972.
Turbotax claims they will walk you through all the assistance you need , but in fact, there is no information. I called Customer Service and the automated line was useless.

The answer below is helpful. I checked my original 1099-R's and my responses to the questions on TurboTax. None of the box 2-b's were checked, nor did I make an error in entering this information. Nonetheless, my taxes were rejected three times when I attempted to submit them electronically. I could not find any indication on my finished form to suggest that they were filled out incorrectly. I have mailed them in; I am hopeful that they will be accepted.
Thank you for your reply, but there appears to be nothing I could have done about this. It's too bad that TurboTax did not provide this information directly, despite their cheery assertions that they will help you find the error that caused the rejection. They were useless.
A lump-sum distribution would be reported on a form 1099-R. If you had one, you would see a check mark in box 2(b) Total Distribution. If so, it may qualify for special averaging which is reported on form 4972. So, if you have an entry in box 2(b) for total distribution, you may need to include the form 4972 with your tax return.
Also, you may have capital gains reported on your form 1099-R, in box three, that may also require form 4972 to be submitted.
If you have these entries on your 1099-R form then you should see the form 4972 included in the copy of your tax forms that you can then use to mail in your tax return.

Did you e-file your tax return and was it accepted?
You complete your tax return by finishing all 3 Steps in the File section. In Step 3, to e-file your tax return, you must click on the large button labeled "Transmit my returns now".
After completing the File section and e-filing your tax return you will receive two emails from TurboTax. The first email when your tax return was transmitted and the second email when the tax return has either been accepted or rejected.
