I received a payment request from IRS for $4521 and would like to understand why this was not caught.

Since you are posting from a "Live" version  you can arrange for the Live help you are paying the extra fee for with your questions from  5 a.m. to 5 p.m. Pacific time Monday - Friday.

If the request is for a payment related to your 2018 return and you used the "Live" services you are posting from, then contact a "Live" expert as already suggested.  But you have provided no details at all--not even what tax year the request is for.  No one in this user forum has access to your account or your tax returns, so without details we cannot give you any help.  Does the letter have a letter number or CP number?  What tax year is it for?  It must say something more than "Pay us $4521."  We cannot guess what your letter says; TurboTax does not get any information from the IRS after you file your tax return.  We do not know what was "not caught."  Did you leave out some income when you prepared your return?
