I used my HSA visa card to buy Glasses at Costco, do I need to report this ?

You will receive a Form 1099-SA reporting all of the distributions from your HSA account during the year.  You will report this on Form 8889 and indicate that the expenses were used for "qualified medical expenses" so that the distributions will not be taxable.

They should send it out automatically, but if you haven't received it yet I would definitely give them a call.
