I need to pay my Massachusetts state tax. When I completed the return, there was a form to print to send in the payment, but now I cannot find it. How do I access this?

The form that you need is the Massachusetts Income Tax Payment Voucher (Form PV). Follow the steps below to obtain the form in TurboTax Online, you can also print the voucher here http://www.mass.gov/dor/docs/dor/forms/inctax16/f1-nrpy/pv.pdf.

• Sign into your Online account

• Go to My TurboTax at the top left

• Select My Tax Timeline

• Scroll down to "Some Things You Can Do"

• Select Download all forms and worksheets.

• Your return will open in Adobe Reader. Click the "printer" icon to open the Print window (or select Print from the File menu in the upper left corner).

As an alternative, you can pay the MA taxes due online at the following Massachusetts Department of Revenue website http://www.mass.gov/dor/pay-your-tax-bill-by-credit-card/pay-your-tax-bill-by-credit-card.html.

are we supposed to simply put the voucher in the envelope with the check, along with the remaining tax forms?
