The form that you need is the Massachusetts Income Tax Payment Voucher (Form PV). Follow the steps below to obtain the form in TurboTax Online, you can also print the voucher here http://www.mass.gov/dor/docs/dor/forms/inctax16/f1-nrpy/pv.pdf.
- Sign into your Online account
- Go to My TurboTax at the top left
- Scroll down to "Some Things You Can Do"

Select Download all forms and worksheets.
- Your return will open in Adobe Reader. Click the "printer" icon to open the Print window (or select Print from the File menu in the upper left corner).
