I didn't receive a form for my 401k. It's through my work and comes out of my paycheck. Should I receive anything to file my taxes?

You do not have to report anything for your 401(k) unless you have taken a disbursement. You contributions to your 401(k) should already be reported in Box 12 on your W-2 with a code "D".
