If you choose to mail in the return in the FILE tab then when you print the return for filing there will also be a cover sheet  with the instructions you seek.

Yes.  Include one original copy of your W-2.  Keep your copy with your tax papers.  You will need to send in any forms that have withholding on them.  Sign your return and mail it to the address listed on the instruction sheet.
