Do I need to mail in my 1040 if I never mailed it in before making adjustments to my taxes? Or do I only mail in the new 1040X?

You can follow the instructions here for the 1040-X. If you choose to mail the 1040-X, you would only need to mail the 1040-X.

Yes. The IRS and state already have a copy of your 1040 when you completed the return. If they need any additional documents, they will send you a letter by mail requesting those.
