My father and mother lived in New York. My father worked for a life insurance company. The company gave him stock during a demutualization in the year 2000.
They moved to Washington state, a community property state. My father died in 2017. My mother had the stock reissued in her name only so that she could sell it., which took place in 2018. I know the price on that date and of course the sales amount but what is the basis, and how do I determine it. I don't know how the stock had his or their names prior to her changing it to hers only.

Your mother's cost basis is the market value of the stock on your father's date of death. You can do an internet search for a site that will look up historical stock prices. Here is a link to one site: http://www.marketwatch.com/tools/quotes/historical.asp
