I had to get a "wage and tax statement" from the IRS. The form did not include the "state income tax" (box 17) withholding amount, can you tell me how to obtain this?

It's a W-2.

You can get this from your employer or, for a fee, from the Social Security Admin.

You should be able to contact your State for that information.

You can get a copy of your W2 from Social Security for $86
