I have an IRA that I took a qualified distribution from.
The IRA is from a 401k that I rolled over three years ago.
What answer do I choose when asked "Where Is This Distribution From?"
The word "Other" is confusing me in the second choice. Otherwise
I'm pretty sure I chose "Other IRA distribution"
Pension rollover account
Other IRA distribution
Neither of the above

As you describe it the distribution was definitely from the IRA.
I expect that you received a 1099-R detailing this transaction. There is a check box inside of Box 7 that says IRA/ SEP SIMPLE. If it is checked, you should not get that question.
This is how to post a 1099-R
Login to your TurboTax Account
Click "Wages & Income" on the top
Scroll down to "IRA, 401(k), Pension Plan Withdrawals (1099-R)"
Answer "Yes" to the question "Did You Have Any of These Types of Income?"
Click "I'll Type it Myself"
Choose "Form 1099-R, Withdrawal of Money from 401(k) Retirement Plans, Pensions, IRAs, etc."
Click "Continue"
From here TurboTax will walk you through entering your Form 1099-R
Once you complete the first one you can Add Another 1099-R

Odd that Box 7 that says IRA/ SEP SIMPLE was checked and I still got that question.
I also checked that box and am getting a screen that says "where is this distribution from?" I marked none of the above but do I leave the distribution amount box blank since I didn't take the distribution? I just rolled it over.
It has the state distribution amount
any withholding
and the state ID no
Do I leave all blank if I didn't receive any cash?
""where is this distribution from?" " - Is this question in reference to a state? IN the 1099-R interview, there are a lot of questions that concerns states.
Or does your screen look like this:
If you did a rollover, and the distribution was from a traditional IRA, then check the first radio button, and proceed to this screen:
Check that you rolled over the money...
My screen looks different. I think I figured it out, but it was confusing. It didn't ask me if I rolled it over into a traditional, just asked if it was a Roth, which I said no to. Then it asked which state it came from and how much.
Yeah, that screen was from the desktop product. There are often three or four variations of screens based on the product.
But now it sounds like it was a state screen inserted in the federal 1099-R interview, to find out if the pension/annuity/IRA distribution should be adjusted for the state.
